چکیده

To summarize current knowledge regarding mechanisms of radiation-induced normal tissue injury and medical countermeasures available to reduce its severity. Advances in radiation delivery using megavoltage and intensity-modulated radiation therapy have permitted delivery of higher doses of radiation to well-defined tumor target tissues. Injury to critical normal tissues and organs, however, poses substantial risks in the curative treatment of cancers, especially when radiation is administered in combination with chemotherapy. The principal pathogenesis is initiated by depletion of tissue stem cells and progenitor cells and damage to vascular endothelial microvessels. Emerging concepts of radiation-induced normal tissue toxicity suggest that the recovery and repopulation of stromal stem cells remain chronically impaired by long-lived free radicals, reactive oxygen species, and pro-inflammatory cytokines/chemokines resulting in progressive damage after radiation exposure. Better understanding the mechanisms mediating interactions among excessive generation of reactive oxygen species, production of pro-inflammatory cytokines and activated macrophages, and role of bone marrow-derived progenitor and stem cells may provide novel insight on the pathogenesis of radiation-induced injury of tissues. Further understanding the molecular signaling pathways of cytokines and chemokines would reveal novel targets for protecting or mitigating radiation injury of tissues and organs.

Residual DNA double strand breaks correlates with excess acute toxicity from radiotherapy

Introduction: A high risk for development of severe side effects after radiotherapy may be correlated with high cellular radiosensitivity. To enhance radiation therapy efficiency a fast and reliable in-vitro test is desirable to identify radiosensitive patients. The aim of present study was to identify the mechanism of radiation induced DNA double-strand breaks (DSBs) and DSB r...

Genotype-phenotype association of TGF-β1 and GST with chemo-radiotherapy induced toxicity

Background: Normal tissue toxicity continues to remain as a major challenge for radiation oncologists for delivering the total dose to the tumour cells in cancer patients. Cellular, molecular and plasma based early biomarkers to predict the overreactions and non-overreactions of normal tissue toxicity before the initiation of radiotherapy can be valuable for personalised treatment. The aim of t...

Tumour radiobiology beyond fractionation

Historically it has been shown repeatedly that single high doses of radiation do not allow a therapeutic differential between tumor and critical normal tissues but dose fractionation does. The purpose of conventional dose fractionation is to increase dose to the tumor while preserving normal tissue function. Tumors are generally irradiated with 2Gy dose per fraction delivered daily to a more or...

Plant medicine in the treatment of radiation-induced normal tissue lesions-A review

Radiation-induced normal tissue lesions, once seen as a consequence of loss of proliferative integrity of clonogenic target cells, inevitable and untreatable are now viewed as a result of dynamic interactions between multiple cell lines within a particular tissue that can be intervened. Normal tissue damage is the most important limiting factor in radiotherapy and a major component in the medic...

Principles of chemoradiation: theoretical and practical considerations.

Chemotherapy agents known to enhance the effects of radiation in preclinical studies have been used concurrently with radiotherapy in numerous clinical trials with the prospect of further enhancing radiation-induced local tumor control. While some success in several tumor histologies has been achieved using this approach, a major concern has been enhancement in normal tissue toxicity. This brie...
